To add a form from the person profile.
Open the relevant Person Profile.
From the Event drop-down menu, click Add Form.
In the Select Template field, enter the template name, then click the search icon or expand the categories on the left to view the available templates.
Select the required template, then click Next.
If required, to add people to a form configured as send-to-many, click the search icon and search for the person.
From the Assign to me, in the team drop-down menu, select the required team.
To assign a different caseworker, from the or Select Team drop-down menu, select the relevant team.
From the then Select Caseworker drop-down menu, select the new caseworker.
Depending on the configuration, you may also need to enter or select the Start Date and Completion Date or select the Allocate Caseworker to case checkbox if the form is part of a screening process.
Click Accept.
